Abstract
Apparatus for the surgical removal of tissue is disclosed comprising a handpiece having a resonant vibrator with a magnetostrictive stack and connecting body encompassing a hollow elongated tool which is ultrasonically vibrated at its tip longitudinally to a peak to peak stroke of at least 0.005 inches at about 25,000 cps (25 KHz). A generator powers the vibrator and is automatically controlled at the frequency to maintain the resonant vibration. Aspiration is provided by suction through the hollow tool at its tip, and a manifold enclosing the tool end of the handpiece provides irrigation.

An ultrasonic surgical aspirator having an improved irrigation/aspiration manifold and components to facilitate quick connections of the irrigation and aspiration lines to a handpiece. In combination with a handle assembly having an ultrasonic vibrator for vibrating an ultrasonic tip and a nosecone, a nosecone extension defining a rotating joint for connection to the nosecone in an articulating joint for connection to a rigid or flexible flue is provided. The nosecone extension further defines an irrigation cowl having an irrigation port for receiving an irrigation connector for connecting the irrigation fluid to the nosecone extension for passage of irrigation fluid through the nosecone extension and a flue about the surgical tip. An aspiration tubulation connector adapted to securely fit a slot through the nosecone and nosecone extension connects an aspiration port on the tip to the aspiration tube for connection to a pump into a discard area. An alternative manifold assembly includes a nosecone extension adapter defining a rotating joint for connection to the nosecone in a cylindrical portion for connection to an extender sheath, the opposite end of which is connected to a flue adapter defining an articulating joint for connection to a flue. Such an alternative manifold assembly is particularly adapted for use in elongated and curved tips with extenders. The detailed structure of the aspiration tubular connector and the irrigation connector is also shown.

An ultrasonic surgical aspirator having an improved irrigation/aspiration manifold and components to facilitate quick connections of the irrigation and aspiration lines to a handpiece. The nosecone extension further defines an irrigation cowl having an irrigation port for receiving an irrigation connector for connecting the irrigation fluid to the nosecone extension for passage of irrigation fluid through the nosecone extension and a flue about the surgical tip. An aspiration tubulation connector adapted to securely fit a slot through the nosecone and nosecone extension connects an aspiration port on the tip to the aspiration tube for connection to a pump into a discard area. An alternative manifold assembly includes a nosecone extension adapter defining a rotating joint for connection to the nosecone in a cylindrical portion for connection to an extender sheath, the opposite end of which is connected to a flue adapter defining an articulating joint for connection to a flue. Such an alternative manifold assembly is particularly adapted for use in elongated and curved tips with extenders. The detailed structure of the aspiration tubular connector and the irrigation connector is also shown.

4425115 - Ultrasonic resonant vibrator

An ultrasonic resonant vibrator has a connecting portion connecting an ultrasonic vibration transducer to a tool which thereby ultrasonically vibrates at one end for fragmenting contacted tissue. The connecting portion is bifurcated by a slot, and a mount for mounting the vibrator in the handpiece is positioned toward the transducer from the slot. This structure is shaped, dimensioned and made of materials having acoustic properties such that there is only one node of ultrasonic vibrational movement between and including the slot and the mount and this node is spaced from the mount toward the slot to obtain increased ultrasonic vibration at the tissue-contacting end of the tool for better fragmenting a wider variety of tissues.

An ultrasonic cutting tip assembly for an ultrasonic surgical cutting instrument having an ultrasonic transducer. The assembly includes a transition horn which is operatively couplable to the transducer, an elongated ultrasonic vibration-transmitting tube which is separable from the horn, and a threaded retaining nut. The threaded portion of the nut threadably engages the threaded portion of the transition horn to hold the proximal end of the tube and the horn together so that ultrasonic vibrations can thereby be efficiently transmitted from the transducer to the distal end of the tube, and then to the surgical site. The nut threaded portion can be formed either on the outside or the inside of the nut, and when on the outside in one embodiment, the nut and tube can be formed as an integral member to be threaded into the distal end of the horn. The elimination of all exposed tip nut surface angles greater than forty-five degrees increases the hydrodynamic performance of the assembly.
